Connect Plus Therapy recognizes that employees may have a family medical emergency or be affected by a major disaster, resulting in a need for additional time off in excess of their available sick/personal time. To address this need, all employees will be allowed to donate accrued paid sick or paid time off hours from their unused balance to an emergency fund. The PTO fund will act as an emergency pool of hours that employees can request extra PTO time from in the case of an emergency. Emergencies can include medical emergencies, bereavement, or disaster.

• Medical emergency, defined as a medical condition of the employee or an immediate family member that will require the prolonged/extended absence of the employee from duty and will result in a substantial loss of income to the employee due to the exhaustion of all paid leave available.
• Bereavement, defined as the loss of a family member or friend, which requires the employee to be absent from work.
• Major disaster, an employee is considered to be adversely affected by a major disaster if the disaster has caused severe hardship to the employee or to a family member of the employee that requires the employee to be absent from work.

This policy is strictly voluntary.

If you’d like to donate PTO hours, use the donation form above. Once you’ve submitted your donation, the hours will be deducted from your available balance and added to the emergency fund.

If you need emergency PTO hours please use the form above. If Human Resources has determined that your request meets the guidelines, the requested PTO hours, if available, will be added to your available balance.

Please Note

At the end of the year, up to 40 hours of sick time can carry over, and PTO hours do not carry over. If you have hours that you know you will not use, please donate them to the pool. Emergency fund hours will carry over from year to year.
